Introduction

Rubicin is used for the treatment of blood cancer (leukemia). It works by interfering with the growth of cancer cells, which are eventually destroyed. Rubicin is given under the supervision of a healthcare professional and should not be self-administered. It might be given as a single therapy or in combination with some other medication. Your doctor will decide what dose is necessary and how often you need to take it. This will depend on what you are being treated for and may change from time to time. You should take it exactly as your doctor has advised. Taking it in the wrong way or taking too much can cause very serious side effects. It may take several weeks or months for you to see or feel the benefits but do not stop taking it unless your doctor tells you to. You should avoid drinking alcohol as it may increase your risk of liver damage. The most common side effects of this medicine include nausea, vomiting, low platelet count, ulcer, loss of appetite and difficulty while breathing. If they do not go away or get worse, tell your doctor. There may be ways of preventing or reducing these effects. It is very strong medicine and some people may develop serious side effects while taking it. This medicine may lower your ability to fight infections and lead to problems with your blood, liver or kidneys. Your doctor will advise you regular blood tests to check for these. Before taking it, tell your doctor if you have liver, or kidney problems or are taking any medicines to treat infections. Many other medicines can affect, or be affected by, this medicine so let your doctor know all medications you are using. This medicine is not recommended during pregnancy or while breastfeeding. It may harm your baby. You and your partner should avoid becoming pregnant or fathering a child for several months after your treatment with it has stopped. Your doctor may perform several tests such as blood tests, ECG, and physical examinations both before and during treatment with this medicine.

How Rubicin works

Rubicin is an anti-cancer medication. It works by damaging the genetic material (DNA) of the cancer cells and stops their growth and multiplication.

Quick Tips

  • You must undergo careful examination of functioning of the heart as daunorubicin is known to cause life-threatening heart problems due to damage to heart muscles (myocardium).
  • Daunorubicin should be used cautiously is elderly, children, and patients with high blood pressure (manifest arterial hypertension) due to increased risk of developing heart complications.
  • You may be frequently examined for complete blood-count (blood cells, proteins, hemoglobin), kidney, liver and heart function before and during the course of treatment.
  • Tell your doctor if you have had, have an ongoing or scheduled radiation therapy or X-ray of the chest area. Daunorubicin increases the risk of reactions at the site of radiation therapy (or risk of heart problem if the chest area is involved) and should be used cautiously.
  • You should be monitored for worsening of infections and development of life-threatening infections (e.g. viral infections like herpes zoster) as daunorubicin is known to suppress the activity of the immune system.
  • Liposomal preparation of daunorubicin is not recommended for use in children <18 years.
  • Before starting daunorubicin treatment, make sure that you are free from any side effects of previous chemotherapy (inflammation of the mouth, abnormal blood cell counts and generalized infections).
  • Both partners should take extreme care to avoid pregnancy while on daunorubicin therapy and consult your doctor if you get to know that you are pregnant.

Indication

Acute leukaemia, AIDS-related Kaposi's sarcoma

Administration

IV Preparation Reconstitute 20 mg vial with 4 mL SWI to a final concentration of 5 mg/mL IV Administration Vesicant, never administer IM or SC IVP: desired dose is withdrawn into a syringe containing 10-15 mL NS, then injected over 2-3 min into the tubing or sidearm of a freely flowing IV infusion of NS or D5W Has also been diluted in 100 mL of D5W or NS & infused over 30-45 min Flush with 5-10 mL of IV solution before & after drug administration

Adult Dose

Acute Nonlymphocytic Leukemia In combination with cytarabine 100 mg/m²/day IV for 7 days first course, for 5 days subsequent courses <60 years old: 45 mg/m² IVP days 1, 2, 3 first course; days 1, 2 subsequent courses >60 years old: 30 mg/m² IVP days 1, 2, 3 first course; days 1, 2 subsequent courses Acute Lymphocytic Leukemia 45 mg/m² IVP days 1, 2, 3 Hepatic Impairment < 1.2 mg/dL serum bilirubin: Dose adjustment not necessary 1.2-3 mg/dL serum bilirubin: 75% of regular dose > 3 mg/dL serum bilirubin: 50% of regular dose

Contraindication

Heart failure. Pregnancy, lactation.

Mode of Action

Daunorubicin forms a stable complex with DNA and interferes with the nucleic acid synthesis. It is a cell-cycle nonspecific agent, but its cytotoxic effects are mostly marked in the S-phase. It also has immunosuppressant and antibacterial effects.

Precaution

Regular blood count and ECG monitoring; elderly, children. Hepatic or renal impairment may increase risk of toxicity. Pre-existing cardiac disease and previous treatment with doxorubicin. Myocardial toxicity leading to potentially fatal congestive heart failure may occur during therapy or mth to yr after therapy cessation. Incidence of myocardial toxicity increases after total cumulative dose exceeds 400-550 mg/m2 in adults, 300 mg/m2 in children >2 yr, or 10 mg/kg in children <2 yr. Risk of severe myelosuppression leading to infection or haemorrhage. Lactation: excretion in milk unknown/not recommended

Interaction

Increased risk of cardiotoxicity when used with cyclophosphamide. Increased risk of hepatic toxicity when used with hepatotoxic drugs e.g. high-dose methotrexate. Potentially Fatal: Immunisation with live vaccines is not recommended. Concurrent radiation may lead to increased radiation reaction.
