প্রশাসন

খাবারের সাথে বা খাবার ছাড়া নেওয়া যেতে পারে। জিআই অস্বস্তি কমাতে খাবারের সাথে নেওয়া যেতে পারে। প্যারেন্টেরাল ডোজ ডিপ আইএম বা স্লো আইভি ইনজে 3-5 মিনিটের বেশি বা IV ইনফিউশনের মাধ্যমে। পুনর্গঠন: IM: inj বা NaCl 0.9% inj-এর জন্য যথাক্রমে 500 মিলিগ্রাম বা 1 গ্রাম শিশিতে 2 মিলি বা 4 মিলি জীবাণুমুক্ত জল যোগ করুন। IV: inj এর জন্য 5 mL জীবাণুমুক্ত জল যোগ করুন, 500 mg শিশিতে dextrose 5% inj, NaCl 0.9% inj বা অন্যান্য উপযুক্ত সলন যোগ করুন। পুরো সলন ভালো করে নেড়ে নিন।

প্রাপ্তবয়স্ক: PO সংবেদনশীল সংক্রমণ 1-2 গ্রাম/দিন 2-4 বিভক্ত মাত্রায়। সর্বোচ্চ: 4 গ্রাম/দিন। সার্জিক্যাল প্রফিল্যাক্সিস 1-2 গ্রাম প্রি-অপ। পরবর্তী ডোজ প্রয়োজন অনুযায়ী দেওয়া যেতে পারে। ত্বক এবং ত্বকের গঠন এবং শ্বাসযন্ত্রের সংক্রমণ: সাধারণ ডোজ প্রতি 6 ঘন্টায় 250 মিলিগ্রাম বা প্রতি 12 ঘন্টায় 500 মিলিগ্রাম। লোবার নিউমোনিয়া: 500 মিলিগ্রাম প্রতি 6 ঘন্টা বা 1 গ্রাম প্রতি 12 ঘন্টা। মূত্রনালীর সংক্রমণ: সাধারণ ডোজ প্রতি 12 ঘন্টা 500 মিলিগ্রাম। গ্যাস্ট্রো-ইনটেস্টাইনাল ট্র্যাক্ট সংক্রমণ: 500 মিলিগ্রাম দিনে তিন থেকে চার বার। IV/IM গুরুতর সংক্রমণ 2-4 গ্রাম/দিন 4টি বিভক্ত ডোজে। সর্বোচ্চ: 8 গ্রাম/দিন।

শিশু ডোজ

শিশু: মৌখিক: স্বাভাবিক মোট ডোজ হল 25 থেকে 50 মিলিগ্রাম/কেজি/দিন 2 থেকে 4টি সমানভাবে বিভক্ত ডোজে। ইনজেকশন: 50 থেকে 100 মিলিগ্রাম/কেজি/দিন 4টি সমানভাবে বিভক্ত ডোজ। স্বাভাবিক মোট ডোজ 200-300 mg/kg/day পর্যন্ত বাড়ানো যেতে পারে। পেরিওপারেটিভ প্রফিল্যাক্সিস: প্রস্তাবিত ডোজ হল 1-2 গ্রাম ইন্ট্রামাসকুলার বা শিরাপথে; পরবর্তী প্যারেন্টেরাল বা মৌখিক ডোজ উপযুক্ত হিসাবে দেওয়া হয়।

রেনাল ডোজ

কিডনি প্রতিবন্ধকতা: দীর্ঘস্থায়ী হেমোডায়ালাইসিসের মধ্যে থাকা রোগীদের জন্য: সেশনের শুরুতে 250 মিলিগ্রাম দেওয়া যেতে পারে, 6-12 ঘন্টা পরে পুনরাবৃত্তি করা যেতে পারে, তারপর আবার 36-48 ঘন্টা প্রাথমিক ডোজ পরে এবং আবার পরবর্তী হেমোডায়ালাইসিসের শুরুতে পূর্ববর্তী ডোজ থেকে 30 ঘন্টা অতিবাহিত হয়েছে। CrCl (ml/min) >20 500 mg প্রতি 6 ঘণ্টায়। প্রতি 6 ঘন্টায় 5-20 250 মিলিগ্রাম। <5 250 মিলিগ্রাম প্রতি 12 ঘন্টা।
