Overview

Overview

The Islet Cell Antibody (ICA) test is used to detect autoantibodies against the islet cells of the pancreas. These autoantibodies are primarily associated with type 1 diabetes mellitus (T1DM) and are indicative of an autoimmune attack on the insulin-producing beta cells in the pancreas.

Risk Assessment

Risk Assessment

  • Positive ICA test indicates an increased risk of developing type 1 diabetes.
  • Individuals with ICA but without diabetes may be in the preclinical phase of type 1 diabetes.
  • In family members of T1DM patients, the presence of ICA can indicate a higher risk of developing the disease.
  • ICAs are often present in latent autoimmune diabetes in adults (LADA), which is a slow-progressing form of autoimmune diabetes.

Normal Range

Normal Range

  • Negative (<1:4 titer or no detectable ICA) – No significant autoimmune response.
  • Positive (>1:4 titer) – Presence of islet cell antibodies, which may indicate risk for type 1 diabetes.

Note: The exact reference range may vary between laboratories.

Interpretation

Interpretation of Results

  1. Negative ICA

    • No detectable islet cell autoantibodies.
    • Low likelihood of autoimmune diabetes.
  2. Positive ICA

    • Indicates autoimmune activity against pancreatic islet cells.
    • Suggests an increased risk of developing type 1 diabetes.
    • Often used in combination with other autoantibody tests (e.g., GAD-65, IA-2, ZnT8) to assess risk.
