Description

The Venereal Disease Research Laboratory test (VDRL) is a blood test for syphilis and related non-venereal treponematoses that was developed by the eponymous US laboratory. The VDRL test is used to screen for syphilis (it has high sensitivity), whereas other, more specific tests are used to diagnose the disease.

Overview

The Venereal Disease Research Laboratory (VDRL) test is a non-treponemal test used to screen for syphilis, a sexually transmitted infection caused by the bacterium Treponema pallidum. It detects the presence of antibodies (specifically, anti-cardiolipin antibodies) produced in response to the infection.

Risk assessment

High-Risk Groups:

  • Individuals with multiple sexual partners.
  • Men who have sex with men (MSM).
  • Individuals with a history of STIs.
  • Pregnant women (routine screening recommended).
  • Persons living with HIV.

Low-Risk Groups:

  • Individuals in monogamous relationships.
  • Those practicing safe sex.

Ranges

The VDRL test result is typically reported as:

  • Negative: No significant antibodies detected.
  • Positive: Indicates the presence of antibodies, which may require further testing (e.g., treponemal tests like the FTA-ABS).

Test result interpretation

Interpretation

  1. Negative Result: No evidence of syphilis.

  2. Positive Result: Further investigation is needed, as a positive VDRL can also indicate other conditions (e.g., certain autoimmune diseases, infections).

    • Titer Levels: If positive, the result may include a titer (e.g., 1:1, 1:4), indicating the concentration of antibodies. Higher titers suggest active disease or recent infection.
  3. False Positives: Conditions such as malaria, pregnancy, and certain autoimmune disorders can cause false-positive results.

Sample types

Serum: Blood is drawn from a vein and serum is separated for testing.
