Medicine Overview of Acaril 50mg Tablet
বাংলাIntroduction
Acaril 50 is used to treat people with type 2 diabetes mellitus. It helps lower the blood sugar levels after meals. It belongs to a group of medicines known as alpha-glucosidase inhibitors. This medicine is usually prescribed in combination with other diabetes medications. Acaril 50 should be taken just before a meal or along with the first mouthful of food. The dose and how often you need it will be decided by your doctor so that you get the right amount to control your blood sugar levels. It may be increased gradually. Do not stop taking it without consulting your doctor. If you do, your blood sugar levels may increase and put you at risk of serious complications like kidney damage and blindness. This medicine is only part of a treatment program that should also include a healthy diet, regular exercise, and weight reduction as advised by your doctor. Monitor your blood sugar levels regularly while taking this medicine. The most common side effects of this medicine include flatulence (gas), stomach pain, and diarrhea. Please consult your doctor if these bother you or do not go away. This medicine by itself does not cause hypoglycemia (low blood sugar levels). However, when used in combination with other diabetes medicines (especially insulin or sulphonylureas), it can lead to hypoglycemia (low blood sugar levels). Such episodes of hypoglycemia should be treated by taking plain glucose. Table sugar or common sugar will not help correct low blood sugar levels if you are taking this medicine. Before taking it, let your doctor know if you have any kidney or liver problems. Do not take this medicine if you have ulceration or inflammation in the bowel (eg. ulcerative colitis or Crohn's disease). Pregnant or breastfeeding women should also consult their doctor before taking it. Please tell your doctor about all the other medicines you are taking to make sure you are safe. Avoid excessive alcohol intake while taking this medicine as it may increase the risk of hypoglycemia (low blood sugar levels).

How Acaril 50 works
Acaril 50 is an anti-diabetic medicine. It inhibits the intestinal enzymes responsible for breaking complex sugars into simple sugars such as glucose. This prevents blood glucose levels from rising very high immediately after meals.
What if you forget to take Acaril 50?
If you miss a dose of Acaril 50, skip it and continue with your normal schedule. Do not double the dose.

Dosage Adult: PO Initial: 25 mg/day q8hr. May adjust gradually. Maintenance: 50-100 mg 3 times/day. Max: <60 kg: 50 mg 3 times/day. >60 kg: 100 mg 3 times/day. Hepatic Impairment Severe
Administration
May enhance effects of other antidiabetics including insulin. Diminished effects w/ GI adsorbents (e.g. charcoal) and digestive enzyme preparations containing carbohydrate splitting enzymes (e.g. amylase, pancreatin). Neomycin and colestyramine may enhance effects of acarbose. May inhibit absorption of digoxin.
Adult Dose
Monitor LFT every 3 mth for 1st yr and periodically thereafter especially for doses >50 mg tid. Reduce dose or withdraw therapy if transaminases elevations persist. If hypoglycaemia occurs, give glucose orally as a 1st aid measure. Adhere strictly to the prescribed diabetic diet to avoid intensification of GI symptoms. Lactation: not known if crosses into breast milk, avoid using in nursing women.
Renal Dose
Acarbose competitively and reversibly inhibits pancreatic alpha-amylase and intestinal brush border alpha-glucosidases, resulting in retardation of glucose absorption from hydrolysed complex carbohydrates and reduction of blood-glucose concentrations.
Mode of Action
Should be taken with food. Take w/ 1st bite of each main meal.

Patients <18 yr; diabetic ketoacidosis or cirrhosis; malabsorption; inflammatory bowel disease; with or pre-disposed to intestinal obstruction; intestinal diseases that affect digestion or absorption; conditions that worsen as a result of increased gas formation e.g. hernias; colonic ulcers; hepatic impairment; CrClr< 25 ml/min/1.73m2. Pregnancy, lactation.
