Dapotin (Dapoxetine Hydrochloride) is a short-acting selective serotonin reuptake inhibitor (SSRI) specifically developed for the treatment of premature ejaculation (PE) in adult men aged 18–64. It helps improve control over ejaculation, reduce distress, and enhance sexual satisfaction.

Indications

• Persistent or recurrent ejaculation with minimal stimulation before or shortly after penetration.

• Marked distress or interpersonal difficulty due to PE.

• Poor control over ejaculation.

Use only under the supervision of a registered physician.

Pharmacology

• Mechanism: Inhibits neuronal reuptake of serotonin, enhancing neurotransmitter action at synaptic receptors.

• Absorption: Rapid, with peak plasma concentration in 1–2 hours.

• Bioavailability: ~42%.

• Protein Binding: >99%.

• Metabolism: Extensive via N-oxidation, N-demethylation, hydroxylation, glucuronidation, and sulfation.

• Elimination: Primarily via urine; half-life ~19 hours.

Dosage & Administration

• Adults (18–64 years): Start with 30 mg, taken 1–3 hours before sexual activity.

• Maximum frequency: Once every 24 hours.

• Dose may be increased to 60 mg if needed and tolerated.

• Can be taken with or without food.

• Not recommended: Children, adolescents, elderly (65+), severe renal or hepatic impairment.

Interactions

• CNS active drugs: Use with caution.

• PDE5 inhibitors (Sildenafil, Tadalafil): May reduce orthostatic tolerance.

• Alpha-blockers (Tamsulosin): Caution advised.

• Warfarin: Limited data; caution required.

• Alcohol: Increases risk of dizziness, drowsiness, syncope—avoid use.

Contraindications

• Hypersensitivity to dapoxetine.

• Significant cardiac conditions (heart failure, conduction abnormalities, ischemic disease).

• Concomitant use with MAOIs, thioridazine, SSRIs, SNRIs, TCAs, or serotonergic herbal products.

• Moderate to severe hepatic impairment.

Side Effects

• Common: Nausea, dizziness, dry mouth, headache, diarrhea, insomnia.

• Rare: Loss of libido (<1%), erectile dysfunction (<4%).

• Discontinuation rates increase with higher doses.

Precautions & Warnings

• Avoid recreational drugs (risk of serotonin syndrome, arrhythmia, hyperthermia).

• Avoid alcohol (risk of syncope, impaired judgment).

• Risk of syncope and orthostatic hypotension—monitor patients.

• Caution in patients with bleeding disorders or on anticoagulants.

Overdose Effects

• No reported cases.

• Symptoms may include serotonin-mediated reactions: somnolence, nausea, vomiting, tachycardia, tremor, agitation, dizziness.
