Indication

Atomoxetine hydrochloride is a medication often used to treat attention deficit hyperactivity disorder (ADHD). It can improve concentration while also reducing impulsiveness and hyperactivity.

Contraindication

Mode of Action

Atomoxetine is known to be a potent and selective inhibitor of the norepinephrine transporter (NET),2 which prevents cellular reuptake of norepinephrine throughout the brain, which is thought to improve the symptoms of ADHD. More recently, positron emission tomography (PET) imaging studies in rhesus monkeys have shown that atomoxetine also binds to the serotonin transporter (SERT),9 and blocks the N-methyl-d-aspartate (NMDA) receptor,10 indicating a role for the glutamatergic system in the pathophysiology of ADHD.

Precautions

Suicidal Ideation and Behaviour: Suicidal ideation (suicidal thoughts) was statistically more frequently observed in clinical trials among children and adolescents treated with Atomoxetine (5/1357; [0.37%]) compared to those treated with placebo (0/851; [0%]). There was one report of suicidal behaviour in a patient in this age group treated with Atomoxetine compared with no reports in patients treated with a placebo. Aggressive Behaviour or Hostility: Although there is no conclusive evidence that Atomoxetine causes aggressive behaviour or hostility, during clinical trials these events were more frequently observed in children and adolescents treated with Atomoxetine compared to those receiving placebo (overall risk ratio of 1.33- not statistically significant). Seizures: The pre-clinical and clinical trial data for Atomoxetine do not suggest that Atomoxetine is proconvulsive. However, seizures have been very rarely reported in post-marketing spontaneous reports. Atomoxetine should be used with caution in patients with a history of seizures. Cardiovascular Effects: Atomoxetine can affect heart rate and blood pressure. It is recommended that the heart rate and blood pressure be measured before treatment is started and periodically during treatment to detect possible clinically important increases.

Side Effect

This is very common: blood pressure and heart rate have increased.Common symptoms include abdominal pain, rash, dyspepsia, early morning awakening, ejaculatory dysfunction, ejaculatory failure, weariness, lethargy, tachycardia, paraesthesia, and chills. Uncommon: peripheral coldness.

Pregnant or nursing women should not use unless the possible benefits greater than the risk to the fetus or the child.

Interaction

Atomoxetine may inhibit Abacavir excretion, resulting in a greater serum level. Atomoxetine metabolism can be improved when coupled with Abatacept. Atomoxetine metabolism can be inhibited when taken with Abiraterone. Acebutolol can reduce Atomoxetine metabolism.Atomoxetine with Aceclofenac may enhance the risk or severity of hypertension.
