Introduction

Extals 50 is a prescription medicine used in the treatment of amyotrophic lateral sclerosis (ALS), a degenerative disease that weakens muscles and hence affects the overall physical function. Extals 50 modifies the action of glutamate, a chemical messenger involved in the transmission of nerve signals in the brain. This medicine is taken on an empty stomach, either 1 hour before or 2 hours after meals. If you have missed a dose, take it as soon as you remember. Do not skip any doses and finish the full course of treatment even if you feel better. It is important that this medication is not stopped suddenly without talking to your doctor. How Extals 50 works

Extals 50 is a glutamate antagonist and used an an anticonvulsant (anti fits). It is believed to work by inhibiting the action of glutamate (chemical messenger) and also inactivates the voltage-dependent sodium channels. In this way it regulates the transmission of nerve signals in the brain.

  • Take it twice a day, 1 hour before or 2 hours after the meal, preferably at the same time.

Administration

Should be taken on an empty stomach. Take on an empty stomach at least 1 hr before or 2 hr after meals.

Adult Dose

Oral Amyotrophic lateral sclerosis Adult: 50 mg bid. Discontinue if ALT levels increase to 5 times the upper limit of normal (ULN). Hepatic impairment Mild or moderate (Child-Pugh A or B): Patients had increases in AUC; thus, may be at increased risk of adverse reactions Severe (Child-Pugh C): Unknown Not recommended for patients with baseline serum aminotransferases (AST/ALTs) >5x ULN or evidence of liver dysfunction (eg, elevated bilirubin)

Contraindication

Hepatic impairment (baseline transaminases >3 times the upper limit of normal).

Mode of Action

Riluzole is a glutamate inhibitor used to slow disease progression and prolong survival rate in patients with amyotrophic lateral sclerosis. The exact mechanism of its action is not fully elucidated but is shown to inhibit the release of glutamate, inactivate voltage-dependent Na channels, and interfere with intracellular events following binding of transmitter at excitatory amino acid receptors.

Interaction
