Therapeutic Class: Oral retinoid for severe acne

Indications (উপযুক্ততা):
Isotrin capsules are prescribed for severe forms of acne, including nodular or conglobate acne, or acne at risk of permanent scarring, which does not respond to standard systemic antibacterials and topical treatments.

Pharmacology (ফার্মাকোলজি):
Isotretinoin works by regulating the cell cycle, differentiation, survival, and apoptosis of sebocytes. This reduces sebum production, prevents pore blockage, and limits growth of acne-causing bacteria. It does not directly kill bacteria but creates a less favorable environment for them. It may also modulate immune responses and inflammation.

Isotretinoin-এর মাধ্যমে সেবোসাইটের কার্যক্রম নিয়ন্ত্রিত হয়, যা তেল উৎপাদন কমায়, রোমকূপ বন্ধ হওয়া প্রতিরোধ করে এবং ব্রণ তৈরি হওয়া ব্যাকটেরিয়ার বৃদ্ধি কমায়।

Dosage & Administration (ডোজ ও ব্যবহার):

• Capsules should be taken with food once or twice daily.

• Adults (including adolescents and elderly): Start with 0.5 mg/kg/day, can increase up to 1 mg/kg/day. Total cumulative dose usually 120–150 mg/kg over 16–24 weeks.

• Children <12 years: Not recommended.

• Severe renal insufficiency: Start at lower dose (e.g., 10 mg/day) and adjust.

Contraindications (বর্জনযোগ্যতা):

• Pregnant or breastfeeding women.

• Women of childbearing potential unless strict Pregnancy Prevention Programme is followed.

• Hypersensitivity to isotretinoin or excipients.

• Hepatic insufficiency, severe hyperlipidemia, hypervitaminosis A, concurrent tetracycline use.

গর্ভবতী বা স্তন্যদানরত মহিলা, Isotretinoin-এর প্রতি অ্যালার্জি থাকা রোগী এবং লিভারের সমস্যা বা রক্তের চর্বি অত্যাধিক উচ্চ রোগীদের জন্য নিষিদ্ধ।

Side Effects (পার্শ্বপ্রতিক্রিয়া):

• Very common: Dry skin, cheilitis (dry/cracked lips), pruritus, erythema, back pain, myalgia, eye irritation.

• Common: Headache, epistaxis, conjunctivitis.

• Rare/Very rare: Depression, suicidal ideation, arthritis, alopecia, photosensitivity reactions, severe skin reactions (SJS/TEN), pancreatitis, benign intracranial hypertension.

ত্বকের শুষ্কতা, চুলকানি, ঠোঁট ফাটল, মাথা ব্যথা, পিঠ ও মাংসপেশীর ব্যথা সাধারণ। বিরল ক্ষেত্রে মানসিক সমস্যা, ত্বকের জটিল প্রতিক্রিয়া ও পেশী-হাড়ের সমস্যা হতে পারে।

Precautions & Warnings (সতর্কতা ও নির্দেশনা):

• Female patients must follow strict contraception measures before, during, and 1 month after treatment.

• Regular follow-up visits every 28 days and pregnancy testing for women at risk.

• Avoid sharing medicine, blood donation during and 1 month post-treatment.

• Use lip balm and moisturizer to prevent dryness.

• Avoid intense sun exposure, aggressive chemical peels, and wax depilation during and for months after therapy.

মহিলাদের জন্য গর্ভনিরোধের কঠোর নিয়ম মেনে চলা আবশ্যক। নিয়মিত ফলোআপ এবং মাসিক পরীক্ষা করা প্রয়োজন। ত্বক ও ঠোঁটের শুষ্কতা প্রতিরোধে ময়েশ্চারাইজার ও লিপ বাম ব্যবহার করা উচিত।

Drug Interactions (ঔষধের পারস্পরিক প্রভাব):

• Vitamin A or derivatives may increase adverse effects.

• Topical keratolytics or exfoliative agents may increase local irritation.

• Benzoyl peroxide may reduce topical isotretinoin efficacy.

ভিটামিন A, অন্যান্য টপিকাল এজেন্ট এবং অক্সিডাইজিং এজেন্টের সাথে ব্যবহার সতর্কতার সঙ্গে করতে হবে।

Storage (সংরক্ষণ):

• Store below 30°C, away from light and moisture.

• Keep out of reach of children.

৩০°C এর নিচে, আর্দ্রতা ও আলো থেকে দূরে রাখুন। শিশুদের নাগালের বাইরে রাখুন।
