Description

The 2 Hours After 100gm Glucose Test measures how the body metabolizes glucose two hours after consuming a 100-gram glucose solution. It is primarily used to diagnose Gestational Diabetes Mellitus (GDM) during pregnancy. Blood glucose levels are checked after two hours to assess the body's insulin response and glucose clearance.

Overview

Overview

This test measures how efficiently the body processes glucose after ingesting 100 grams of glucose. It is most commonly used in the evaluation of gestational diabetes mellitus (GDM) and sometimes for confirming impaired glucose tolerance or diabetes mellitus in non-pregnant individuals.

After overnight fasting (8–12 hours), a fasting blood sample is taken. The patient then drinks a solution containing 100 gm of glucose, and blood samples are collected at 1 hour, 2 hours, and sometimes 3 hours after ingestion.

Risk Assessment

Risk Assessment

  • Gestational Diabetes Mellitus (GDM) screening (especially after abnormal 50 gm glucose challenge test).

  • Detection of impaired glucose tolerance.

  • Diabetes mellitus diagnosis confirmation.

  • Monitoring glucose metabolism in high-risk groups (obese, family history, PCOS, etc.).

Normal Range

Normal Range (2 hours after 100 gm glucose)

Category Plasma Glucose (mg/dL)
Normal <140 mg/dL (7.8 mmol/L)
Impaired Glucose Tolerance 140–199 mg/dL (7.8–11.0 mmol/L)
Diabetes Mellitus ≥200 mg/dL (11.1 mmol/L)

Gestational Diabetes Diagnostic Criteria (Carpenter & Coustan method)

Diagnosis made if 2 or more of the following are elevated:

Time Plasma Glucose (mg/dL)
Fasting ≥95
1 hour ≥180
2 hour ≥155
3 hour ≥140

Interpretation

Interpretation

  • Normal Result: Indicates effective glucose utilization and normal insulin function.

  • Elevated 2-hour value (140–199 mg/dL): Suggests impaired glucose tolerance, a prediabetic state.

  • ≥200 mg/dL: Indicates diabetes mellitus.

  • In pregnancy, ≥155 mg/dL at 2 hours suggests gestational diabetes, depending on which criteria are used.
