Overview
Purpose:
Measures the amount of haptoglobin, a plasma glycoprotein produced by the liver that binds free hemoglobin released during red blood cell (RBC) destruction.
Used primarily to help diagnose hemolytic anemia and differentiate it from other types of anemia.
Risk Assessment
- Suspected hemolytic anemia
- Sudden drop in hemoglobin/hematocrit
- Jaundice or dark urine without liver disease
- Reticulocytosis (increased reticulocyte count)
- Monitoring response in autoimmune hemolytic anemia, transfusion reactions, or mechanical heart valve hemolysis.
Normal Range

30–200 mg/dL (varies slightly by lab)

In intravascular hemolysis, levels often fall to <25 mg/dL or undetectable.
Interpretation of Results
| Haptoglobin Level | Clinical Interpretation |
| Low or undetectable | Suggests intravascular hemolysis (haptoglobin bound to free hemoglobin and cleared) |
| Normal | Can be seen in extravascular hemolysis or non-hemolytic conditions |
| Elevated | Seen in acute-phase reactions, inflammation, infection, or tissue injury |
Note: Liver disease may also lower haptoglobin levels (due to reduced synthesis), so correlate with LFTs.
